Credit card must be on file prior to booking an appointment. If you need to cancel your appointment, we require a minimum of 24 hour notice prior to your scheduled appointment or there will be a $50 charge for no show no call. This $50 fee can be applied as a credit towards your next treatment. Of course we understand there are extenuating circumstances that occur causing last minute cancelations, we will only allow two of these per calendar year. Our time and our loyal patient’s time is valuable to us.
2 week follow up is required after neurotoxin if (1) you are a new patient to toxin all together or (2) are a new patient to our practice. We have this policy in order to better serve you! This opportunity allows us to make any modifications, document outcomes and speak to you regarding the results. If this appointment is not canceled within 48 hours of appointment, credit card on file will be charged $50 fee. This $50 fee can be applied as credit towards your next treatment.
